Adult Self Assessment
Describe your needs to access information, advice and signposting personalised to you. If you need further support you will then have the option to send your self assessment on to us.
The assessment (which is free) will consider:
- what your main needs are
- what strengths and resources you are able to draw on
- what options are already available to you to support these needs
- how you can best be supported to live as independently as possible
- your home and how you are coping in it
- your emotional wellbeing
- your diet
- any health and medical treatments you need
Getting an assessment
If it looks like you need social care support, we will offer you an assessment. You can choose how to do this by:
- completing a self assessment via our Better Care portal
- book a telephone appointment to speak to a advisor

You can tell us about your needs in your own words by filling in an online form.
A friend or family member can help if you agree, or if they have Lasting Power of Attorney.
We use this information to guide you to the right advice and support.
