Transition overview

Information about the transition overview.

We have identified activities and resources that support effective transition for teachers, pupils, and parent carers. We have suggested when these might be used in Key Stage 2 and 3 but many can be used at other transition points.

Year 5

Teacher activities:

  • pyramid subject leader meeting focusing on key vocab. KS2 > KS3
  • secondary staff visit Primary
  • secondary staff attend Year 5 PEP/AR (mainstream children)

Parent experiences:

  • parents to attend secondary open evening
  • include parent feedback

Pupil experiences:

  • familiarisation of potential new setting
  • Pupil Voice – My thoughts about secondary

Teaching or learning activities:

  • RSHE transition materials delivered in Year 5

Year 6

Teacher activities:

  • secondary staff visit Primary
  • training of parent or carer ambassadors to support with workshops
  • year 7 training of pupil ambassadors
  • 1 March National Offer Day
  • document sharing (SEND Co, Heads of Year, DSL)
  • secondary assembly with Year 6 pupils and Year 7 pupil ambassadors

Parent experiences:

  • open evenings or mornings
  • coffee morning in Primary for SEND parents with Primary and Secondary SEND Co
  • document shared with parents ‘How to help your child’s transition’
  • secondary staff offer transition surgeries (drop in coffee mornings for parents who are worried about their children and transition)
  • secondary staff offer transition workshop – practical sessions to upskill parents to support their child’s worries
  • parents meet key pastoral staff – policies shared (wellbeing, BEH, SEND, etc)

Pupil experiences:

  • familiarisation of potential new setting
  • additional Year 6 visits for vulnerable pupils
    • tour
    • meet key adults
  • Year 6 transition day(s)
    • what does it feel like to attend this school?
    • pupil voice

Teaching or learning activities:

  • RSHE transition materials delivered in Year 6
  • year 6 staff send best piece of writing to secondary

Year 7

Teacher activities:

  • year 7 induction lessons and activities

Parent experiences:

  • parental feedback on transition

Pupil experiences:

  • key adults to be accessible / visible to new pupils
  • year 7 pupil ambassadors to be visible in corridors

Teaching or learning activities:

  • RSHE transition materials delivered in Year 7
  • pupil feedback on transition
  • Pupil Voice – my thoughts about secondary
