Why use the Self‑Assessment tool:

  • helps you understand your needs as the questions follow the same principles used in formal adult social care assessments, including an easy checklist based on the Care Act

When should you use it?

Use the Better Care Support Self‑Assessment if:

  • you want to know what help might be available for you or someone you care for
  • you’re unsure whether you need a full Adult Social Care assessment
  • you want to explore support options online before contacting us
  • your situation is not urgent, but you’d like advice on staying independent, safe, and well

If the tool suggests you may have eligible needs, you have a legal right to a full Care Act assessment. Our Adult Social Care team will work with you to understand what matters most and how best to support you.

What happens next?

After completing the self‑assessment, you may be:

  • directed to local community services
  • given advice to support your wellbeing
  • guided to start a full Care and Support Assessment
  • signposted to information about independence, reablement, home support, or assistive technology
