What is an Individual Support Plan?
A support plan is a simple written document that explains how a setting will support a child with additional needs.
It outlines:
- The child’s strengths and areas of need
- The outcomes we are working towards
- The support and strategies in place
- Who is responsible for delivering the support
- How progress will be reviewed
Support plans are part of the graduated response (assess, plan, do, review). They help ensure that support is clear, consistent and focused on helping the child make progress.
They are created in partnership with parents and, where appropriate, other professionals, so that everyone is working together to meet the child’s needs.
