When to Use:

- annually or at regular intervals as part of quality assurance or self-evaluation
- after significant changes in staffing, policies, or service delivery that may impact parent communication
- when introducing new programs or initiatives that involve parent participation
- if feedback from parents indicates areas for improvement in communication, involvement, or support
